If you are requesting an appointment for a doctor visit or a flu vaccine, please fill out this form and we will contact you as soon as possible to schedule. We will contact you to schedule in the order in which requests are received. Please only fill out this form one time per patient.
Please note, any patient who fails to show for an appointment and has failed to cancel or reschedule within a 24 hours’ notice will be considered a no-show and will be charged a $25 no-show fee. Any patient who fails to show for a 2nd consecutive appointment and has failed to cancel or reschedule an appointment within 24 hours’ notice will be considered a no-show and will be charged a $50 no-show fee.
